GENERAL DFA PASSPORT REQUIREMENTS:

1. Personal Appearance

3. Fully Filled Up Application form. Click [Here] to download.
4. Birth Certificate
5. Valid IDs and supporting documents to prove your identity.

You may provide any (1) of the following ID.
SSS ID, [Driver’s License], GSIS e-Card, PRC ID, IBP ID, BIR ID, Senior Citizen’s ID etc.

Other Acceptable picture IDs:
Old college ID, Alumni ID, Old Employment IDs

List of Supporting Documents:
PSA Marriage Contract, Land Title, Seaman’s book, elementary and high school form 137, Government Service Record, [NBI Clearance], Barangay Clearance, Digitized Postal ID, School yearbook.

6. Marriage Contract – for married women only.

Passport Application without Birth Certificate:
If born AFTER 1950, proceed to LCR or Local Civil Registrar and apply for late registration and submit the original copy of the PSA authenticated late registered Birth Certificate with the requisite supporting documents and IDs that pre-date the late registration.

If born ON or BEFORE 1950, submit copies of PSA Authenticated Certificate of No Birth Record and Affidavit of Two Disinterested Persons attesting to the identity of the applicant.

DFA PASSPORT REQUIREMENTS FOR MINORS / 18 YEARS OLD AND BELOW

1. Confirmed Appointment. Not applicable for 7 years old and below.
2. Accomplished Application Form
3. Personal Appearance
4. For Legitimate Child – Personal appearance of the applicant and EITHER parent are required plus the passport of both parents
4. If Illegitimate Child – Personal appearance of both applicant and the mother, plus valid IDs or passport of the mother. If the mother cannot accompany her child during the application process, a Special Power of Attorney is required and must be authenticated by the Philippine Embassy/Consulate if executed abroad.
5. Birth Certificate
6. Documents of Identity. For 8- 17 years old *may provide a school ID or form 137 with readable dry seal.
7. Parent’s Marriage Certificate
8. Passport copy of the person traveling with the minor or applicant.
 
ADDITIONAL DOCUMENTS:
Not Travelling with either parent or alone:
1. Personal Appearance of either parent.
2. Affidavit of Support and Consent
3. Original and photocopy of DSWD Clearance.

DFA PASSPORT APPLICATION FEES

For First Time Passport Applicant, Passport Renewals, Replacement of lost, expired passport or valid passport:
Php 950.00 Regular Processing / Php 1,200.00 Express Processing

Regular Processing Timeframe: 15 days or 2 weeks
Express Processing Timeframe: 7 days or 1 week
